At a Glance:Formed in 2008, the Inland Empire Long-Term Services and Supports (IE LTSS) Coalition brings together the expertise of diverse health care entities, community-based organizations, public officials, and stakeholders to better serve older adults and people with disabilities in San Bernardino and Riverside Counties. The Coalition is committed to facilitating systems change across networks and developing improved integrated care and service delivery of long-term services and supports.

Over the next two decades, California’s over-65 population will nearly double, with those aged 85 and older being the fastest growing demographic group. While this change in demographics will introduce new opportunities for economic and community growth, it will simultaneously lead to an increased demand for health and support services.
In January 2021, the State of California began to implement a new Master Plan for Aging (MPA), a 10-year blueprint for state government, local government, the private sector, and philanthropy to prepare for these coming demographic changes. The plan was informed by a network of aging and disability providers, policy makers, advocates, and experts, and addressed topics such as housing, healthcare, community-based services, social isolation and loneliness, equity and inclusion, caregiver support, and financial security for older adults. To complement the statewide plan, the California Department on Aging recognized the importance of engaging communities to develop their own, locally-informed Master Plans for Aging. The SCAN Foundation provided financial support to three advocacy coalitions, including the Inland Empire Long Term Support Services (IE LTSS) Coalition, to focus on developing regional Master Plans for Aging that address the unique challenges of aging in a rural community. Over the next two years, the IE LTSS Coalition will engage an Advisory Committee comprised of representatives from the Riverside and San Bernardino County Offices on Aging, Inland Empire Health Plan, Inland Caregiver Resource Center, Community Access Center, Alzheimer's Association, housing and transportation leaders to spearhead this effort. |

The Community Access Center (CAC) co-leads the IE LTSS Coalition with the Inland Empire Health Plan. CAC is an independent living center with a mission to empower persons with disabilities to control their own lives, create an accessible community and advocate to achieve complete social, economic, and political integration. CAC implements this vision by providing information, supportive services and independent living skills training.
